Appointment booking — collection of a residence card

Before booking an appointment, please make sure that the residence card is ready for collection at the relevant Branch Office where the decision was issued, i.e. in Częstochowa, Katowice or Bielsko-Biała. This information can be obtained by calling the helpline at 780 100 935.

Appointments will be automatically cancelled if:

- the residence card has not yet physically arrived at the relevant Branch Office;

- a decision has not yet been issued in the case of the person for whom the appointment was booked;

- the person entered in the booking is fictitious or is not listed in the Office’s records;

- several appointments have been booked for the same person.

Therefore, before making a booking, please call the helpline at 780 100 935 to confirm that the card is ready for collection. When booking an appointment through the online queue management system, please provide the correct and complete first name and surname of the person collecting the card, as well as their email address.
