The foreigner is obliged to submit:
If you apply for a temporary residence permit for the purpose of family reunification you should submit:
- documents, recognized by Polish law, confirming the degree of yours relationship (a marriage certificate, a birth certificate);
- a residence card or Polish administrative decision conforming that the foreigner with whom you are making a family has right to stay legally in Poland (a copy to deliver and the original available for inspection)
- documents confirming possessing a health insurance or coverage by the insurer of the cost of medical or ambulatory treatment in Poland (e.g. ZUS – Polish Social Insurance Institution - certificate, other insurance policy - for instance: ZUS RCA report signed by an authorised person);
- documents confirming that you have got a source of stable and regular income sufficient to cover the costs of living for yourself and your family members dependent on you (e.g. employment contract, employment and salary certificate)
- documents confirming that you have got a permanent dwelling place (e.g. permanent residence certificate, residence lease agreement, residence lending contract, any other agreement giving you the rights to housing premises or someone else’s declaration to provide you a permanent dwelling place in Poland).
In case you apply for a temporary residence permit due to the necessity to respect the right to a family life, as defined by the Convention on Protection of Human Rights and Fundamental Freedoms, you should submit:
- documents confirming you have got a health insurance to cover costs of hospital and ambulatory treatment on the territory of Poland (e.g. an applicable certificate from ZUS - Polish Social Insurance Institution, insurance policy etc.)
- Documents confirming that you are making a family life
- documents confirming that you have got a source of stable and regular income sufficient to cover the costs of living for yourself and your family members dependent on you (e.g. employment contract, employment and salary certificate)
In case you apply for a temporary residence permit for a child born in Poland, you should submit:
- The child's birth certificate
- Documents proving that the child’s parents have got a legal temporary residence permit or visa;
- documents confirming possessing a health insurance or coverage by the insurer of the cost of medical or ambulatory treatment in Poland (e.g. ZUS – Polish Social Insurance Institution - certificate, other insurance policy);
- documents confirming possessing having a stable and regular source of income sufficient to cover the costs of living for yourself and your family members dependent on you (e.g. employment contract, income and salary statement)
- documents confirming that you have got a permanent dwelling place (e.g. permanent residence certificate, residence lease agreement, residence lending contract, any other agreement giving you the rights to housing premises or someone else’s declaration to provide you a permanent dwelling place in Poland).
In case you apply for the temporary residence permit and you are a foreigner who is a family member of a citizen of Poland, European Union, European Economic Area, CH who is living in Poland you need to submit:
- documents confirming possessing a health insurance or coverage by the insurer of the cost of medical or ambulatory treatment in Poland (e.g. ZUS – Polish Social Insurance Institution - certificate, other insurance policy);
- documents confirming possessing having a stable and regular source of income sufficient to cover the costs of living for yourself and your family members dependent on you (e.g. employment contract, income and salary statement)
- Documents, recognized by Polish law, confirming the degree of yours relationship (a marriage certificate, a birth certificate);
- evidence of common residence with a family member in the other country,
- evidence confirming the financial dependency
- evidence confirming the necessity of personal care of a foreigner
- a certificate of residence registration or a document confirming permanent residence permit of a family member.
