Who can file an application ?

A document confirming the right of permanent residence san be issued to a citizen of a Member State of the European Union who after 5 years of continuous stay in Poland acquires the right of permanent residence.

Documents

In order to receive a document confirming the right of permanent residence of EU citizen, the following documents should be submitted:

• A completed application form (Application form is available below) for issuance of the document confirming the right of permanent residence of an UE citizen + 3 copies
• 5 recent photographs - intact, in colour, measuring 35 x 45 mm, made in the past 6 months, against a light background, having good focus, clearly showing the eyes and face from the top of the head to the top of the shoulders, so that the face occupies 70-80% of the photo; photograph should show the person without a hat and dark glasses, looking straight ahead with eyes open, not covered by hair, with a natural facial expression and mouth closed;
• valid travel document or another document confirming your identity and citizenship (for review) + 2 copies
• documents confirming the 5-year continuous residence in Polish territory.
